Provide training support for Clark County Primary Care Provider groups currently participating in Washington State Diabetes Collaborative. |
Result: |
Over 450 diabetes patients reduced their risk factors associated with this chronic disease; 22 primary care providers received diabetes education training and over 650 patients are being tracked on a Diabetes Registry. |
